Treatment Protocol

Standard International protocol when patient presents between 0- 2 years of Age

  • 0-3 months – pre surgical orthodontics
  • 3 months – cleft lip nose repair
  • 1 year cleft palate repair
  • 5 years ? Pharyngoplasty/redo palate for speech if needed
  • 7-11 years secondary alvelour bone grafting
  • 13 years orthodontics
  • 17 years orthognathic surgery
  • 17 years rhinoplasty
  • Distraction osteogonisis at any age

Protocols for  repair of cleft deformities presenting  between 2 – 7 years of age is as follows:

  • 2 to 7 years: palatoplasty first and then lip repair followed by standard international protocol
  • 7 years to adulthood: palatoplasty first and then lip repair with simultaneously alveolar bone grafting followed by standard international protocol
  • When a bilateral cleft patient presents for surgery after the age of 7 years, the premaxilla may be severely displaced and may require surgical repositioning.
